The Unemployment Compensation Program administered by the Florida Department of Economic Opportunity (DEO) is now called the "Florida Reemployment Assistance Program."

Form 1099-G showing Florida unemployment/reemployment benefits should be mailed to you by January 31st following the end of the previous calendar year. The form is mailed to your last official address of record on your reemployment claim. If you change your address and there is an active forwarding address on file with the United States Post Office, the form should be forwarded to your new mailing address.

Click here to access the Florida Reemployment Assistance Program (RAP). Once you enter your Social Security number and PIN and successfully login, click on the menu item on the left side of the webpage that says "My 1099-Gs and 49Ts". You will then be able to select the tax year and print your Form 1099-G. If you repaid any reemployment benefits, you will also be able to print Form 49T.

To request a duplicate Form 1099-G for the current year or previous years by e-mail, you will need to send an e-mail to: 1099G@deo.myflorida.com. The e-mail request MUST include your complete name, ONLY the last four digits of your Social Security number, previous mailing address on your claim, county of residence, new mailing address, and the specific calendar year(s) requested for Form 1099-G.

NOTE: Under Florida law, e-mail addresses are public record. If you do not want your e-mail address released to the public, do NOT send an e-mail. Instead, contact the Florida Department of Economic Opportunity by telephone, fax, or postal mail.

The fax request MUST include your complete name, ONLY the last four digits of your Social Security number, previous mailing address on your claim, county of residence, new mailing address, the specific calendar year(s) requested for Form 1099-G, and your signature.
